Home health care provides medical services in the comfort of a patient’s home, supporting recovery and improving health for homebound individuals recovering from surgery or managing illness, injury, or chronic conditions.
Home care is paid for by Medicare and commercial insurance (with approval).
When a client qualifies for home care services, Medicare will cover 100% of home care services (i.e., Nursing, Physical Therapy, Home Health Aide, all other therapies, and medical social services) and 80% of the approved amount for Durable Medical Equipment (i.e., wheelchairs, hospital bed, walker, glucometer). Most private insurances also cover qualified home care services.

You can receive home care by making a request and will make an initial visit to assist you in determining your plan for home health care. This visit is done at no cost to you.

You can call (708) 444-2300 anytime. Office hours are M-F 9 a.m. - 5:30 p.m. Weekends and Holidays are covered by an answering service and the on-call nurse that nurse is available 24 hours a day, 7 days a week.

A qualified staff member will be able to evaluate your request and will make an initial visit to assist you in determining your plan for home health care. This visit is done at no cost to you.

If you would like to change to another health care provider, you have the right to do so. You must make it known to the agency, so they can discharge you before our new service starts.

Yes. However, there may be times when it is necessary to use others to cover scheduled leave and illnesses.

Our staff is all interviewed, references are obtained, and criminal background checks are done. A thorough orientation is done at the time of hire. Our staff is licensed by the Illinois Department of Public Health and the City of Chicago, bonded and insured.

Only if your doctor has ordered skilled care to be done such as nursing, physical therapy, occupational therapy, medical social work, or speech therapy.
For patients unable to leave home, our doctors offer in-home visits tailored for your needs.
